NOTICE OF WITHDRAWAL

Once a student has been given a lesson time, it is assumed they will continue lessons indefinitely at Saied Music Studios.

To terminate your lessons, Saied Music Studios requires two weeks’ notice and must be notified via this Withdrawal Form. Billing will cease two weeks following Saied Music Studios’ receipt of the Withdrawal Form.
